We can also assist during the transition home from a hospital or rehab, as well as work along-side your skilled home health care or hospice care team to maintain continuity of care and comfort. Our care begins with a Free Assessment by a Licensed Nurse who will assist the client and family with deciding the level of care that works best for the individual. Our team of home care professionals consist of both licensed and non-licensed caregivers, including Companion Aide, Homemaker, Home Health Aide, CNA, LPN and RN. Our caregivers have each successfully completed an in-depth screening process and skill evaluation at hire, and must maintain ongoing education and credentials throughout the course of their employment, to ensure that our clients are receiving the best in quality care. We are qualified to work with many community organizations and private insurances to help offset the out of pocket cost of private care. Including, but not limited to local Area Agencies on Aging, Veteran’s Affairs, Long Term Care Insurance, Auto Insurance and Worker’s Compensation. Private home care is not typically covered by health insurance, Medicare or Medicaid.
